The forms may be filled out on-line. One you have completed all the required fields on the form, you will be given the option to "Finish". Once you click "Finish", your forms will be automatically submitted to us. You will be given the option to download the form or have it emailed to you as well.
If the patient is under 18 years of age, please complete the Authorization To Treat a Minor form along with the New Patient forms.
